We lean in on how "problems identified" do and don't manifest as "lessons learned".  To truly be a lesson learned, the investigation must either validate an existing policy or change must be affected.  Otherwise, it is simply a problem identified that lies in wait.

Motivations matter!  Discipline matters more!  The discipline to do the right thing, at the right time, bringing the right people and stuff, to simply do the right thing.  Through a series of case study's we discuss how the normalization of deviance manifests in our organizations and discuss ways to keep today's bad habits from becoming next weeks normalizatin of deviance.
